In lieu of a signature, the applicant agrees to the following Liability Waiver.

Liability Waiver: Payment of dues means you agree to the following waiver. I (we) do hereby agree to indemnify and hold the City of Sunnyvale and Sunnyvale Tennis Club from and against any and all liability for any injuries which may be incurred by me (us) arising out, or in any way connected with, my (our) participation in any event sponsored by the aforementioned.
